The tissue is destroyed when an inflammatory breakout destroys collagen. The skin over this section is left unsupported, resulting in a depressed area. Boxcar scars range in severity depending on the amount of tissue lost.

Ice Pick Scars

Ice pick scars are deep, narrow scars that reach the dermis. The skin appears to have been punched by an ice pick or other sharp instrument. Ice pick scars penetrate the skin in a small, thin, deep hole. Some may appear to be a large, open pore. Ice pick scars form when an infection from a cyst or other deeply inflamed blemish rises to the surface. Skin tissue is destroyed, resulting in a long, columnar spot.

Hypertrophic and Keloid Scars

Hypertrophic scars are firm, raised spots that grow above the skin’s surface. Acne-induced hypertrophic scars are most commonly found on the torso, particularly in men, but they can occur anywhere on the body. After a deep wound or trauma, hypertrophic scars are more common.7

Unlike boxcar scars and ice pick, Hypertrophic scars are not caused by tissue loss. Rather, they develop as a result of excessive collagen production. In the case of keloids, the skin appears to be unaware that the wound has healed and continues to produce collagen.
